Patient-Centered Care

At Columbia Medical Associates we believe in delivering high quality care for every member of your family.

A patient-centered medical home provides customized care to fit your health needs throughout every stage of your life. At Columbia Medical Associates, our patients have ready access to a wide range of medical providers, assuring care that is comprehensive, coordinated, and convenient. You and your loved ones will receive personalized, friendly day-to-day care at each of our locations – and your overall health is our focus.

What This Means for You

Personal Care Team. Columbia Medical Associates is dedicated to a team approach when it comes to your health care. In addition to your primary doctor, your team could include a physician assistant, nurse practitioner, nurse, and a medical assistant. Together, you and the members of your health care team will make decisions about your health, including Behavioral Health Services, health education, and care from other certified professionals.

Continuity of Care. Your personal care team is familiar with all your health care needs, even across different facilities within Columbia Medical Associates. With information about your medical history and care received outside of CMA, your team will be fully equipped to address your needs and identify the health care services that you or your loved ones require.

Understanding Your Health Care Journey. At Columbia Medical Associates, we strive to make your health care journey as easy as possible. Part of that process involves constant communication within your care team. In addition, any questions or concerns regarding your health care will be answered and addressed at any point along the way.

Equal Access. We believe in upholding each patient’s right to equal access of health care, regardless of source of payment, ethnicity, race, nationality, religion, gender, age, sexual orientation, marital status, disability, veteran status, or any other basis prohibited by law.
