Community Resource Specialist


Columbia Medical Associates is seeking a Full-time Community Resource Specialist as part of our Behavioral Health Department located at our South Hill Family Medicine clinic.

The Community Resource Specialist functions as in important link between the patient’s primary care team and their surrounding community and empowers patients to get active in their own care. This role develops relationships with community organizations and connects patients with community resources. Coaching patients with low to medium care needs to identify health goals and remove barriers to meeting goals.

  • Experience: One year providing direct services to clients. Experience interacting in a diverse, multicultural and/or multilingual environment.
  • Knowledge/Skills/Abilities: Must be able to collaborate with medical professionals and have the ability to adapt to a fast-paced primary care environment. 
  • Knowledge/Skills/Abilities: Understanding and sensitivity to how socioeconomic, environmental, cultural, and other factors influence health. Self-motivated, good organizational skills and strong communicator.
  • Education: High School Diploma

Preferred :Bachelor’s degree; Health Science related studies.
